Puppy Purchase Contract
Name of Puppy/Date of Birth/Price _______________________________________
Buyer accepts full responsibility for proper care of the puppy once it leaves the breeder. Buyer is subject to maintenance of proper diet, taking the puppy to a licensed vet within 7 days upon arrival of the new home and keeping up to date on all required vaccinations. If at any time during the life of the puppy you cannot keep the puppy, the breeder has the first opportunity to take the puppy back with no refund.
Health Guarantee
Your new puppy is guaranteed by the breeder under the following conditions:
1. The buyer must have the puppy examined by a licensed vet within 7 days of arrival. If during the initial exam your vet should find the puppy to be in poor health you must contact the breeder within 24 hours. Diagnosis of coccidiosis, Giarrdia, or other parasites commonly found in puppies does not render your puppy unhealthy and the breeder is not responsible, as well as uneven bite, hernia, or undescended testes. Any vet bills while the puppy is in your care are your responsibility.
2. If your puppy is found unhealthy you may elect to keep the puppy or exchange for a puppy from a future litter. If you do not seek a vet within 7 days the warranty is void.
3. If within the first year of the puppy’s life the puppy receives a certified diagnosis as having a congenital defect including heart murmur, epilepsy, or hip dysplasia, you may elect to keep the puppy or exchange it for a puppy from a future litter.
4. Guarantee is voided in the event of an indeterminable or inconclusive diagnosis. Guarantee does not include any illness due to the ingestion of foreign objects, chemicals or physical injury, carelessness, neglect or abuse.
If one of the above conditions exists within the exception of the voids previously stated, the breeder will gladly honor its guarantee by replacing the puppy with a new puppy upon written statement by a certified vet. Buyer may elect to keep the puppy, but will be fully responsible for any vet bills. A certified diagnosis is defined as a positive diagnosis by a veterinary hospital or state licensed D.V.M Acceptable to the breeder’s own veterinarian.
